Direct Primary Care Basics

  • Direct Primary Care (DPC) is a membership-based healthcare model where patients pay a flat monthly fee for unlimited access to their primary care physician. This model removes the hassles of insurance, allowing for longer appointments, direct communication with your doctor, and more personalized care.

  • DPC offers:
    Unrushed, longer visits – More time with your doctor to discuss your health.
    Same-day or next-day appointments – No waiting weeks for an appointment.
    No copays, no surprise bills – Transparent and predictable costs for primary care.
    Direct access to your doctor – Text, call, or video chat when needed.
    A focus on prevention & wellness – Comprehensive care beyond just treating symptoms.

  • No. While both models offer personalized care and direct physician access, concierge medicine typically bills insurance AND charges an additional retainer fee, whereas DPC does not bill insurance at all and operates solely on an affordable monthly membership.

  • No, DPC is not the same as health insurance. DPC is a healthcare model, not an insurance plan. You still need health insurance for hospitalizations, surgeries, and specialist care. Think of DPC as an affordable way to access primary care without the middleman.

    DPC covers all of your primary care needs, but you should still have insurance for major medical events like hospitalizations, surgeries, and emergencies. Many members pair DPC with a high-deductible or cost-sharing plan for catastrophic coverage.

  • In the DPC model, we provide care to our members without the restrictions and frustrations associated with insurance. We will not file a claim with your insurance which means you will not need to pay a copay for your visit.

  • Yes! Medicare patients can join Empowered Health DPC, but due to federal regulations, it's important to understand that:

    • Medicare will not pay for your DPC membership.

    • We will NOT submit a claim to Medicare, and you should not attempt to get Medicare to cover the cost of your DPC membership.

    • You can still use your Medicare benefits for specialists, hospitalizations, imaging, and prescriptions.


    • Enjoy longer visits, direct physician access, and a more personalized approach that Medicare-based clinics often can’t provide due to insurance constraints.

  • At Empowered Health DPC, I use a variety of tools to help you navigate any diagnosis. As a family medicine physician, I am trained in comprehensive, evidence-based care across a broad range of conditions. My most valuable tool is time—time to listen, understand your concerns, research the best treatment options, and provide you with the highest level of personalized care.


    I also have access to a number of platforms that allows me to collaborate with board-certified specialists, including psychiatrists, dermatologists, cardiologists, and orthopedic experts. If your case requires additional expertise, I can securely share your symptoms, medical history, and any diagnostic work (labs, imaging) with these specialists at no extra cost to you—giving us quick expert guidance without unnecessary referrals or delays.


    If you have PPO insurance, I can refer you to any specialist within your network. If you are uninsured, we can utilize specialists, such as cardiologists, ophthalmologists, and dermatologists, who offer transparent cash pricing for their services.


    If you have an HMO insurance plan, I can attempt to facilitate a referral to a specialist within your network. However, HMO referrals often require approval from your designated primary care physician (PCP), and many are denied by HMO administrators. In those cases, you would need to visit the PCP listed on your insurance card for a formal referral. If needed, I can provide a detailed letter explaining the medical necessity of the referral to support your request.


    My goal is to ensure you receive the best possible care—whether through in-depth consultations, virtual specialist input, or direct referrals—while minimizing barriers and delays in your treatment.

Empowered Health DPC

  • Empowered Health serves South Jersey and Philadelphia in 2 clinic locations.

    The main physical location is in the Iron Stone Villages Center located at 560 Stokes Road, Suite 10E, Medford NJ 08055. As you drive into Iron Stone Villages from Stokes Road, drive straight towards the White Gazebo on the left side of the complex. My office is on the second floor of Ironstone Villages. The entrance to the stairs are next to the Mi Pueblito Mexican Restaurant.

    A location has been added in Philadelphia’s Fishtown at 2418 E. York St, Philadelphia, PA in the MedCoShare Suite.

  • Due to restrictive covenants, I needed to establish my clinic a certain distance from my prior location. I value continuity of care and will welcome any of my patients who choose to establish their care with me directly. Dr. Aliganga plans to eventually move from his Medford location closer to Haddon Heights or West Deptford in early spring 2026. In the meantime, I am excited to show you my new (and cozy) clinic office spaces.

  • YES! Telemedicine has become a valuable tool for healthcare providers, making care more accessible, convenient, and efficient. At Empowered Health DPC, we embrace telemedicine and use it to benefit our established members.


    Telemedicine is particularly helpful for:

    • Chronic Disease Management: Telemedicine enables regular check-ins and consultations for patients with chronic conditions.

    • Simple Evaluations: Patients can receive answers to straightforward concerns without visiting our clinic.

    • Mental Health Support: Telemedicine offers comfortable and private spaces for patients seeking mental health care.

    • Check-Ins and Updates: Minor health concerns and medication refills can be addressed, saving time and resources.

    However, in-person visits are sometimes necessary, such as for acute concerns requiring physical exams. At Empowered Health DPC, we offer same-day or next-day sick appointments without additional copays. Our dedicated team will ensure you receive the most appropriate care for your unique needs, whether that means a telemedicine consultation or an in-person visit.


    The direct primary care (DPC) model allows us to provide a higher level of personalized care than traditional healthcare practices. We're committed to accessibility, quality care, and staying at the forefront of healthcare innovation while maintaining a patient-centered approach.

  • Yes and No. You will have access to our personal (members only) phone number and have access 24/7 to ask questions by calling, leaving a voicemail, or sending a text message. The most secure and preferred method of contacting is through our Mobile App/Patient portal SigmaMD.

    For urgent concerns or questions, please call (leave a message if I do not respond to the call as I may be busy with the care of another patient) and I will get back to you as soon as I can. I would also recommend that you follow up with a text.

    For non-urgent medical questions, please utilize your patient portal to send a message (to ensure HIPAA compliance) or email (but be aware that email and texts are inherently less secure). You should get a response within 2 business days (but usually much much quicker).

    For general questions about Empowered Health DPC, membership benefits, etc, please email and we will respond within 1-2 business days.

    Because Empowered Health DPC is a micropractice, Dr. Aliganga will be primarily the only person managing and responding to messages. This allows for more direct communication. However, do not wait to go to the emergency room for evaluation if you are concerned that your symptoms or concerns are worsening.

Joining Empowered Health DPC

  • We do not recommend you cancel your health insurance. You still need health insurance for major medical events like hospitalizations, surgeries and emergencies.

  • For a limited time, the enrollment fee has been waived. After the introductory period, the enrollment fee will be $100.

  • Because establishing care with a new physician requires an investment for you and Dr. Aliganga, we request a 3 month minimum commitment.

    The enrollment fee is waived if you pay for 3 months. However, if you cancel prior to the completion of the initial 3 month commitment, only partial refund will be given. The refund will be prorated but we will retain $100 as an enrollment fee.

    After the 3 month initial period, if you decide to obtain care elsewhere, please provide a 30 day notice.

  • Individual and family health needs and priorities can change and we respect your reason for wanting to cancel.

    If you need to cancel, we require a 30 day notice.

    If you have elected for a quarterly, semi-annual, or annual term and decide to cancel, you will receive your remaining prorated balance.

  • Re-enrollment is not guaranteed and is subject to approval and availability in our patient panel. Re-enrollment fee may apply.
