This policy explains how Empowered Health DPC LLC ("Empowered Health," "we," "us") collects and uses information through our website, www.myhealthempowered.com, and our email newsletter. It does not cover the medical care we provide. Information you share as a patient is handled through our secure patient portal and under the terms of your patient agreement.
1. Information we collect
Information you give us. When you fill out a form, sign up for our newsletter, book a call, or email us, we collect what you provide, such as your name, email address, phone number, and your message.
Information collected automatically. Like most websites, our site records basic visit information, such as the pages you view, your browser and device type, the website that referred you, and your approximate location based on your IP address. This is collected through Squarespace, our website platform.
2. Please don't send medical information through the website
Our website forms and regular email are not meant for medical information. Please don't include symptoms, diagnoses, medications, or other health details. Patients can message us securely through the patient portal, and prospective patients can discuss their health with us privately on a call or at a visit.

4. Email
If you sign up for our emails, we'll send you information about our practice and health topics. Every email includes an unsubscribe link, and you can opt out at any time.

We do not sell your personal information. We share it only with service providers that help us run the practice, and only as needed for them to do that work. These include Squarespace (website, forms, and email), Google Workspace (email), Motion (scheduling), and SigmaMD (patient portal, enrollment, and billing). We may also disclose information if required by law.
6. Cookies
Our site uses cookies that are needed for it to work, plus Squarespace analytics cookies that help us understand site traffic. We do not currently use advertising or retargeting cookies. When you first visit, a cookie banner lets you accept all cookies or choose which ones to allow. You can also block or delete cookies in your browser settings.
7. How we protect information
We use reputable service providers that encrypt information in transit, and we limit access to the people who need it. No website or email system is completely secure, so we can't guarantee absolute security.
8. How long we keep information
We keep information only as long as we need it for the purposes described here or as required by law. You can ask us to delete your information at any time.

You can ask to see, correct, or delete the personal information we hold about you, or unsubscribe from our emails. To make a request, contact us using the details below.
10. Children
Our website is not directed to children under 13, and we do not knowingly collect information from them online. Care for children is arranged directly with a parent or guardian.

We may update this policy from time to time. We'll post the new version on this page with an updated effective date.
